Confianza® Pro (marketed as Conquest Pro in North America) is Asahi Intecc's highest tip-load penetration wire for the most difficult chronic total occlusions — those with dense, organized fibrocalcific proximal caps that resist all lower tip-load approaches. The 9 gf standard grade and 12 gf Confianza Pro 12 variant deliver concentrated point force through a tapered 0.009/0.014 in tip that acts as a needle rather than a blunt probe, allowing direct penetration through calcified cap material when rotating-core wires such as the Gaia series cannot advance. The wire is used within a structured escalation algorithm: Confianza Pro is reached only after Fielder XT, XT-A, and the Gaia series have been attempted. Due to the very high tip force, correct technique is critical — the wire is rotated (not pushed) to avoid vessel perforation, and a supportive microcatheter (Corsair Pro or Caravel) must be present immediately behind to prevent buckling and to provide back-up. Tip load of 9 gf is sufficient for the majority of fibrocalcific cases; the 12 gf grade is reserved for the most calcified, undilatable caps. - Why does Confianza® Pro have an uncoated ball tip?
- The uncoated ball tip design prevents the wire from self-advancing inadvertently once it has penetrated the cap. At 9–12 gf tip load, a fully hydrophilic-coated tip would slide through the vessel wall too easily after initial cap penetration. The uncoated ball provides controlled resistance, giving the operator tactile feedback about where the tip is and preventing unintended deep advancement that could cause perforation.
- What technique is required when using Confianza® Pro?
- Confianza Pro should always be used with a supportive microcatheter (Corsair Pro, Caravel, or SASUKE) immediately behind it. The wire is rotated — not pushed — to drive the tapered tip through the cap. A supportive microcatheter prevents wire buckling, which is critical at high tip loads because a buckled wire can suddenly release stored energy and spring forward. After cap penetration, the microcatheter is advanced over the wire to confirm true-lumen position before exchanging for a softer wire.
