Fielder® XT-A and Fielder® XT-R are second-generation refinements to the Fielder XT CTO guidewire platform, each addressing a specific gap in the original wire's performance. Fielder XT-A (Advanced) stiffens the proximal shaft relative to Fielder XT while keeping an identical tapered tip geometry — the result is improved pushability and column strength once an initial path has been identified, allowing the wire to advance further into the occlusion without the buckling that sometimes occurs when pushing a softer shaft against a resistant lesion. Tip load on XT-A is approximately 1 gf, up from XT's 0.8 gf. Fielder XT-R (Reverse) takes the opposite approach: it softens the tip to approximately 0.6 gf while keeping the same proximal shaft stiffness as XT-A. XT-R is preferred when the antegrade wire has already crossed the occlusion and is suspected to be in a subintimal space — the softer tip allows controlled re-entry into the true lumen with less risk of perforation than a higher-load wire. Both wires share the same 0.010/0.014 in tapered tip, 17 cm polymer + hydrophilic coating, and are available in 190 cm and 300 cm lengths.
