ASAHI Gaia®Product Code (REF): AHW14R007P

The ASAHI Gaia® series introduced a rotating-core (composite core) construction to the CTO guidewire category — a design that physically decouples the outer coil from the inner rotating core, eliminating the torque wind-up and sudden unwinding that causes conventional wires to jump unpredictably when rotated under resistance. The result is precise, linear 1:1 torque transmission even at high tip loads (up to 4.5 gf for Gaia Third), making deliberate dissection and re-entry technique reliably achievable at the tip of a standard guidewire rather than requiring specialised retrograde equipment. The three tip-load grades are matched to escalating lesion difficulty. Gaia First (1.7 gf) handles moderately calcified caps with a fibrous component that resists Fielder XT but doesn't yet require high force. Gaia Second (3.5 gf) is the most commonly used grade for complex antegrade crossing, combining meaningful penetrating force with the controlled torque response the rotating-core construction provides. Gaia Third (4.5 gf) is reserved for dense fibrocalcific caps where Second cannot penetrate; at this tip load, a stiffer shaft and very controlled rotation technique are essential to avoid dissection extension. All Gaia variants use a pre-shape tip, 0.010/0.014 in tapered outer diameter, 40 cm hydrophilic coating, and are available in 190 cm and 300 cm lengths.

Deflection and Torque Control

To guide the wire through the occlusion to the distal lumen by using deflection and rotational control.

Controllability

Within strong resistance area, ASAHI Gaia experiences higher torque response and less whip motion.

Penetrability by micro-cone tip

The ball tip was made smaller to increase its penetration efficacy. It makes easier to create the entry route to the hard tissue and fibrous cap.

Tip flexibility

By taking advantage of tip flexibility, ASAHI Gaia easily avoids hard tissue and advances smoothly. Deflection control: Guiding the wire to the distal true lumen with rotational control by using deflection which occurs when advancing a shaped GW.

Frequently Asked Questions

What is the ASAHI Gaia® series and what makes it different from other CTO wires?
The ASAHI Gaia® series introduced rotating-core (composite core) construction to CTO guidewires. The inner core physically rotates independently of the outer coil, eliminating torque wind-up — the main cause of unpredictable wire tip jumping in conventional CTO wires. This delivers 1:1 torque at the tip even at high tip loads (up to 4.5 gf for Gaia Third), enabling controlled, deliberate antegrade dissection and re-entry in complex CTOs.
What are the tip loads for Gaia First, Second, and Third?
Gaia First: approximately 1.7 gf — for moderately calcified caps where Fielder XT cannot advance. Gaia Second: approximately 3.5 gf — the most commonly used grade, suitable for the majority of complex antegrade crossing cases. Gaia Third: approximately 4.5 gf — reserved for the hardest fibrocalcific caps where Second cannot penetrate.
What are the catalog numbers for the ASAHI Gaia® series?
Gaia First: AHW14R007P (190 cm) and AHW14R307P (300 cm). Gaia Second: AHW14R008P (190 cm) and AHW14R308P (300 cm). Gaia Third: AHW14R011P (190 cm) and AHW14R311P (300 cm). All are pre-shape tip only, 0.010–0.012/0.014 in tapered diameter, 40 cm hydrophilic coating.
When should I use Gaia First versus Gaia Second versus Gaia Third?
Start with Gaia First when Fielder XT-A has failed — Gaia First's 1.7 gf rotating-core tip handles moderately fibrous caps that resist tapered hydrophilic wires. Escalate to Gaia Second (3.5 gf) when First cannot penetrate — this is the workhorse grade for most complex CTO crossing. Use Gaia Third (4.5 gf) only when Second fails against a true fibrocalcific cap; at this tip load, precise rotational technique and microcatheter back-up are essential to avoid perforation.
What microcatheter pairs best with the Gaia series?
ASAHI Corsair Pro is the standard companion for Gaia wires. Its rotatable braided shaft provides back-up support during wire rotation, prevents wire buckling under load, and can advance through the occlusion body independently after wire crossing. For very tight proximal caps, Corsair Pro XS offers a lower crossing profile.
What is the difference between Gaia series and Gaia Next® series?
The Gaia Next series refines the original Gaia coil geometry for smoother tactile feedback and recalibrates the tip load grades upward (Next 1: 2 gf, Next 2: 4 gf, Next 3: 6 gf vs First: 1.7 gf, Second: 3.5 gf, Third: 4.5 gf). Gaia Next 3's 6 gf extends the torque-drive crossing range before needing to escalate to Confianza Pro. Gaia Next is only available in 190 cm; the original Gaia series offers both 190 cm and 300 cm.
