RG3 exists for the final act of retrograde CTO PCI: externalization. Once a retrograde wire and microcatheter have crossed the occlusion, RG3 travels the full retrograde route — donor guide, collateral channel, occlusion, antegrade guide — to create a continuous guide-to-guide rail. Its 330 cm usable length is what makes that journey possible while leaving workable wire outside both guides. The wire is deliberately gentle on the anatomy it transits. The 0.26 mm (0.010 in) shaft — thinner than a standard 0.014 in wire — flexes with the collateral course to help prevent vessel damage, and 170 cm of hydrophilic coating keeps movement inside the retrograde microcatheter smooth over the long overlap. A 3 gf tip with 3 cm radiopaque segment provides the control needed to complete the route under fluoroscopy. Once externalized, RG3 serves as the delivery rail over which antegrade balloons and stents track with exceptional back-up — the reason the retrograde approach, when it succeeds, converts difficult CTOs into deliverable ones.
