V-Grip®: WEB Device Delivery, Repositioning, and Retrieval System

The V-Grip® from MicroVention Terumo is the delivery and repositioning handle for WEB SL and WEB SLS intrasaccular devices, enabling controlled intrasaccular placement, one-handed resheathing for repositioning, and precision pre-detachment positioning before electrolytic release with the WEB Detachment Controller.

What Is the V-Grip®?

The V-Grip® is the dedicated delivery and manipulation handle for the WEB SL and WEB SLS intrasaccular devices from MicroVention Terumo. It is the interface between the operator and the WEB device during the deployment sequence, providing ergonomic one-handed control for advancing, deploying, repositioning, and ultimately releasing the WEB within the aneurysm sac. Without the V-Grip, the fine movements required for precise intrasaccular WEB positioning would be difficult to achieve with standard microcatheter delivery push wires.

The V-Grip works by coupling to the WEB delivery wire at the hub, allowing the operator to control fine forward-and-backward movements of the WEB relative to the VIA microcatheter tip with a precision that push-wire manipulation alone cannot reliably achieve in the intracranial environment. The ergonomic grip design enables one-handed operation so the other hand can simultaneously manage the VIA microcatheter position at the guide catheter hub.

Role in the WEB Procedure

The complete WEB procedure requires three distinct device accessories in addition to the WEB itself and the VIA microcatheter: the V-Grip (for delivery and positioning), the WEB Detachment Controller (for electrolytic release), and the roadmap fluoroscopic imaging system. The V-Grip is used from initial WEB deployment through the pre-detachment positioning phase; it is disconnected before the Detachment Controller is connected for final release.

The V-Grip’s most valuable function is the resheathing capability: if the initial WEB deployment position is not optimal — for example, the device is deployed too deep into the sac or the neck coverage is asymmetric — the V-Grip allows the operator to resheath the WEB into the VIA microcatheter and reposition before re-deploying. This repositioning can be performed multiple times before detachment, providing substantial room for technique optimisation.

Technique Tips

Hold the VIA microcatheter hub with one hand to maintain its intrasaccular position and operate the V-Grip with the other hand. Slow controlled movements with the V-Grip translate to fine adjustments in WEB position within the sac. A 1–2 mm movement at the V-Grip produces the same movement at the WEB tip in the sac.

When resheathing for repositioning, perform the resheathing movement slowly and confirm on fluoroscopy that the WEB is being recaptured into the VIA microcatheter. A sudden decrease in resistance during resheathing may indicate the VIA microcatheter has moved distally into the sac rather than the WEB being recaptured — verify catheter position before re-advancing the WEB.
