Gladius MG adds a modified, prolapse-resistant core to the Gladius polymer platform — keeping the tip aligned with the penetration vector at resistant CTO caps where standard polymer-jacketed wires buckle and fold back on themselves.

The Prolapse Problem in Polymer-Jacketed CTO Wires

Every polymer-jacketed wire faces the same failure mode at a resistant proximal cap: the soft distal segment buckles under forward force and folds back on itself — prolapse. Once the tip prolapses, forward force no longer acts along the intended penetration vector; the folded wire either deflects around the cap into the subintimal space or simply absorbs push without progress.

For the operator this means losing the precise entry point that angiography or IVUS identified, and repeated prolapse cycles enlarge the proximal dissection plane, making subsequent crossing attempts harder.

How the Modified Core Minimises Prolapse

ASAHI Gladius® MG addresses prolapse at the core level: a modified core construction carries forward force closer to the tip before the polymer jacket’s flexibility takes over. The result is a wire that holds its penetration vector at fibrous caps — the tip engages, resists folding, and converts push into forward progress — while retaining the polymer jacket and hydrophilic coating that give the Gladius family its low-friction tracking inside the occlusion body.

This combination lets the operator work a resistant cap with a polymer wire rather than escalating immediately to a stiff penetration wire, keeping perforation risk lower during the engagement phase.

Escalation Position: Between Middle-Weight Polymer and Stiff Wires

In the Asahi CTO escalation sequence, Gladius MG occupies the step where Gladius EX or a tapered polymer wire deflects or folds at the cap, but committing to Gaia Next or Confianza Pro penetration wires is not yet justified. Because it remains a polymer-jacketed wire, de-escalation back to tracking mode after cap penetration is seamless — the same wire follows the occlusion body without exchange.

Gladius MG runs on the standard 0.014 in platform with microcatheter support, in both antegrade and retrograde strategies.
