Guide Catheter Backup in Complex PCI: Why It Matters

Guide catheter backup — the resistance of the guide catheter to being backed out of the coronary ostium as interventional equipment is advanced over the wire — is a critical variable in complex coronary intervention. In complex cases — CTO crossing with stiff wires, delivery of atherectomy devices, placement of covered stents, or advancement of large-calibre equipment in tortuous anatomy — guide catheter disengagement under advancement force can abort the procedure.

High-back-up guide catheters resist this disengagement by providing greater engagement force at the coronary ostium and a stiffer shaft that does not compress under the advancement force of equipment pushed over the wire. The tradeoff is that a stiffer, more deeply engaged guide catheter can cause ostial injury if the tip is not atraumatic.

Hyperion Design: Balancing Backup Force and Tip Safety

ASAHI Hyperion® concentrates rigidity in the proximal and mid-shaft — where backup force is generated — while maintaining flexibility and atraumatic tip geometry at the distal end that engages the coronary ostium. This provides the backup force needed for complex equipment delivery without the risk of ostial dissection that a fully rigid catheter would carry.

The specific tip geometry is designed for stable, coaxial engagement with the coronary ostium across the range of ostial anatomies encountered in clinical practice. Stable coaxial engagement reduces lateral force applied to the ostial wall during equipment advancement, which is the primary mechanism by which high-backup guide catheters cause ostial injury in sub-optimally positioned systems.

Clinical Indications

The Hyperion is indicated for complex PCI cases where standard guide catheters have failed to provide adequate backup, where pre-procedure assessment suggests complex equipment delivery will be required, or where operator preference is for high-backup support as the starting configuration in all complex cases. Specific indications include: CTO PCI with antegrade equipment including microcatheters and stent delivery systems; rotational or orbital atherectomy; left main PCI requiring simultaneous two-stent techniques; and cases referred after failed procedure at another centre due to inadequate guide backup.

Available in multiple tip curve configurations and French sizes to accommodate different coronary anatomies, access sites, and patient body sizes.
