What Is the MIRACLEbros® Series?
MIRACLEbros® is a family of graduated tip-load CTO guidewires from Asahi Intecc, available in three grades: 3 gf, 6 gf, and 12 gf. All three wires use silicone coating rather than a polymer jacket or hydrophilic layer, a deliberate design choice that provides consistent tactile feedback along the entire shaft — operators can feel cap resistance through the wire with greater direct sensation than with hydrophilic or polymer-coated wires, which can mask tissue resistance feedback.
The series predates the Gaia rotating-core platform and remains in widespread use, particularly among operators trained in the classic Asian CTO technique (tiered penetration with escalating tip loads). MIRACLEbros is 0.014 inch in diameter throughout (non-tapered), distinguishing it from Fielder XT’s microchannel-probing tapered design.
The three grades — 3, 6, and 12 gf — correspond broadly to fibrous, fibrous-calcified, and densely calcified cap resistance categories, allowing structured escalation within a single wire family before committing to Confianza Pro. The silicone coating facilitates device advancement while preserving the tactile clarity that experienced CTO operators rely upon for real-time cap characterisation.

How MIRACLEbros Compares to Alternatives
MIRACLEbros vs. Gaia series: The primary differences are core construction (standard vs. rotating) and coating (silicone vs. hydrophilic). In tortuous vessels, Gaia’s rotating core provides superior torque transmission, reducing the prolapse risk that can occur when rotating a standard-core MIRACLEbros 6 or 12 gf wire in a multiply-bent vessel. In relatively straight vessels with fibrous caps, MIRACLEbros 6 gf may be equally effective to Gaia Second (3.5 gf) with the advantage of superior tactile feedback.
MIRACLEbros 12 gf vs. Confianza Pro (9 gf): MIRACLEbros 12 gf has a higher stated tip load but a blunt non-tapered tip, whereas Confianza Pro 9 gf uses a tapered 0.009-inch needle-like tip that concentrates force over a smaller surface area, providing greater penetrating pressure (force per unit area) despite its lower stated load. For truly calcified nodular caps, Confianza Pro’s tapered tip is generally more effective than MIRACLEbros 12 gf.
The silicone coating of MIRACLEbros is its most distinctive attribute: it provides controlled friction that some operators prefer for active wire steering in tortuous vessels, as opposed to the very low friction of hydrophilic-coated wires which can slide unpredictably. This makes MIRACLEbros a wire with sustained relevance even in the era of Gaia and Confianza Pro.
Clinical Applications and Indications
MIRACLEbros 3 gf is appropriate as the first penetration wire after Fielder XT microchannel probing fails, providing approximately four times the tip load of Fielder XT while maintaining manageable tip stiffness. It is effective for soft fibrous caps where Fielder XT deflects rather than penetrates.
MIRACLEbros 6 gf is the workhorse of the series, indicated for fibrous-calcified caps with moderate resistance. In the classic tiered approach, operators move from MIRACLEbros 3 to 6 when the cap resists after 3–5 rotation attempts, then from 6 to 12 if 6 gf is also insufficient. This deliberate stepped escalation allows accurate cap characterisation without committing immediately to the highest tip loads.
MIRACLEbros 12 gf is reserved for heavily fibrotic or mixed fibro-calcific caps that resist 6 gf. If MIRACLEbros 12 gf also fails, the cap is most likely calcified nodular and requires either Confianza Pro’s tapered needle tip or the antegrade subintimal technique. The 12 gf wire is rarely used as a first or second step and serves primarily as a bridge before committing to Confianza Pro.
Technique and Usage Tips
The silicone coating of MIRACLEbros means the wire behaves differently from hydrophilic wires during advancement: expect more friction during balloon and microcatheter tracking over the wire, particularly in calcified vessels. After crossing with MIRACLEbros, consider exchanging to a hydrophilic-coated wire (SION or SION Black) using the over-the-wire exchange technique for subsequent device delivery to reduce friction during balloon and stent advancement.
Tip shaping: manually shape MIRACLEbros to a 30–45° primary bend 1–2 mm from the tip. Silicone-coated wires retain manual tip shapes reliably during the procedure. Avoid bends greater than 60°, which risk kinking the wire at the bend. In very tight calcified caps, a steeper 60° bend occasionally improves cap engagement but increases kinking risk.
When using MIRACLEbros 12 gf, avoid excessive rotation (>3–4 turns in one direction without counterrotation) as the high tip load in a stiff wire increases the risk of torque wind-up and sudden prolapse. Use slow, deliberate quarter-turn rotations with very light forward pressure, allowing the tip load rather than forward force to penetrate the cap.
