SheathLess Eaucath® eliminates the introducer sheath for coronary guiding catheter insertion through the radial artery, reducing total arterial entry profile and enabling a larger inner guide lumen through the same puncture size.

The Sheathless Access Concept: Eliminating the Sheath Wall

Conventional transradial coronary intervention requires an introducer sheath placed in the radial artery before the guiding catheter is advanced. The sheath adds its own wall thickness to the total arterial entry profile — the sheath wall consumes a significant portion of the arterial entry size. Sheathless access eliminates the sheath entirely: the guiding catheter is inserted directly through the arterial wall using a dedicated dilator, reducing the total arterial entry profile to the guide catheter’s own outer diameter.

This allows operators to use a guide catheter with a larger inner lumen through the same arterial puncture that would otherwise be limited by the sheath’s outer diameter.

Clinical Benefits of Sheathless Radial Access

The primary benefit is the ability to use a larger-lumen guide catheter — typically equivalent to a 7F or 7.5F inner lumen — through a radial puncture that without a sheath is effectively equivalent to a 6F standard sheath insertion. This expanded inner lumen accommodates simultaneous two-balloon dilatation in bifurcation cases, orbital or rotational atherectomy devices, and some covered stent delivery systems.

Sheathless access has also been associated with lower radial artery occlusion rates in some series, attributed to the reduced total wall-compression profile during the procedure. Radial artery occlusion forecloses future radial access for that arm and may have implications for patients requiring repeated procedures or radial artery harvest for bypass surgery.

Eaucath Design: Tapered Dilator and Hydrophilic Coating

The SheathLess Eaucath® system includes a tapered dilator that creates a graduated passage through the arterial wall, reducing trauma of direct catheter insertion. The hydrophilic outer coating reduces friction as the catheter is advanced through the radial and subclavian anatomy to the aortic root.

The learning curve for sheathless insertion is steeper than for sheath-based access. Centres adopting this approach for the first time should plan for a supervised training period with operators experienced in sheathless technique. Eaucath is available in standard coronary guide catheter curve configurations and French size equivalents for different coronary anatomies.
