Scepter™ Mini: Small-Vessel Balloon Microcatheter for Distal Intracranial Aneurysms and Sub-3 mm Vessels

The Scepter™ Mini extends the MicroVention Terumo Scepter balloon microcatheter family to intracranial vessels ≤3 mm in diameter, enabling balloon-assisted coiling and selective embolization in M2, distal ACA, PICA, and other small-calibre vessels where standard Scepter C would create dangerous over-distension.

What Is Scepter™ Mini?

The Scepter™ Mini is the small-vessel member of the MicroVention Terumo Scepter balloon microcatheter family, designed for balloon-assisted procedures in intracranial vessels at or below 3 mm in diameter. The standard Scepter C 4 mm balloon would over-distend a 2–2.5 mm M2 vessel even at minimum inflation volumes, creating risk of dissection, spasm, or rupture. The Mini’s balloon profile and compliance characteristics are matched to the 1.5–3.0 mm target vessel range, ensuring effective occlusion at safely controlled inflation volumes.

Like all Scepter variants, the Mini uses a dual-lumen design with a working channel that remains patent during balloon inflation, allowing contrast injection for angiographic assessment of occlusion completeness without balloon deflation. This dual-lumen advantage over single-lumen small-vessel balloon alternatives is particularly critical in small vessels where visual confirmation of occlusion is essential before committing to injection.

Clinical Applications

Balloon-assisted coil embolization of distal aneurysms arising from M2 segments, distal ACA branches (A2/A3), PICA, and other vessels in the 1.5–3.0 mm range. Aneurysms at these locations are less common but technically challenging — the small parent artery calibre makes coil herniation risk high without balloon support, and standard balloon microcatheters are too large.

Selective liquid embolic delivery for distal AVM feeding pedicles where flow arrest is needed in very small vessels (under 3 mm). The Mini provides flow arrest in pedicles where the Scepter C would be too large and a standard balloon microcatheter would create vascular injury risk.

Technique Tips

Navigate to the target distal vessel using standard 0.014 in wire and Headway 17 initial access, then advance the Mini coaxially or over exchange wire. In small distal vessels, the catheter tip is particularly sensitive to torque and push force — use gentle road-map guidance and avoid forcing against wall resistance.

Inflate at the minimum volume required to achieve angiographic flow arrest. Over-inflation in sub-3 mm vessels risks dissection at even slightly supramaximal volumes. If adequate occlusion cannot be achieved at conservative volumes, reassess balloon sizing before increasing pressure.

Scepter™ Mini vs. HyperForm

HyperForm (Medtronic) is the primary competing small-vessel balloon microcatheter, used in similar vessel calibres. The Scepter Mini’s dual-lumen design allowing contrast injection during inflation distinguishes it from HyperForm’s single-lumen design, providing the same angiographic assessment advantage that differentiates the full Scepter family from single-lumen competitors.

For programs already standardised on Scepter C and XC for larger vessels, the Scepter Mini is the natural small-vessel extension that maintains consistent handling and dual-lumen technique across the full range of intracranial vessel diameters.
