What Is the LVIS™ EVO?

The LVIS™ EVO is a next-generation braided nitinol intracranial stent from MicroVention Terumo, engineered as a refinement of the original LVIS and LVIS Jr platforms. Its defining improvement is a tighter correspondence between the delivery catheter working diameter and the deployed stent diameter — a characteristic the engineering team refers to as matched-profile deployment. This closer diameter match translates to more predictable deployment mechanics: the stent engages the vessel wall more smoothly and with less tendency to jump proximally during the final moments of sheath retraction, giving the operator greater control over landing zone accuracy.

The EVO also achieves substantially lower delivery and resheathing forces than the standard LVIS, which is meaningful in daily practice because it reduces the push force needed to advance the delivery system through tortuous access routes and reduces the pull force needed to resheath the device for repositioning. Operators transitioning from standard LVIS to EVO consistently describe the deployment as feeling lighter and more controlled.

Clinical Applications and Indications

LVIS EVO is indicated for stent-assisted coil embolization of wide-neck intracranial aneurysms where the neck-to-parent-artery diameter ratio or dome-to-neck ratio makes primary coiling technically unstable. DAPT with aspirin and clopidogrel must be loaded ≥5 days before the procedure and continued per institutional protocol post-implant.

The EVO’s tortuous-access performance makes it particularly appropriate for posterior circulation cases accessed via a tortuous vertebral artery, and for anterior circulation cases with pronounced carotid siphon angulation. Programs that have experienced LVIS delivery failures in difficult anatomy should trial the EVO as the first-line device for future difficult-access cases.
