What Is the VIA™ Family?
The VIA™ microcatheter family is a set of purpose-designed delivery catheters from MicroVention Terumo created exclusively for WEB intrasaccular device deployment. Unlike the Headway family — which serves as a general microcatheter for coils, stents, and flow diverters — the VIA catheters are validated only for WEB deployment. Their inner diameters, tip profiles, and deployment mechanics are optimised for the WEB device geometry, not for standard coil or stent delivery.
VIA microcatheter selection is matched to WEB device diameter: VIA 17 for WEB SL/SLS 3–7 mm; VIA 21 for 7–8 mm; VIA 27 for 8–9 mm; VIA 33 for 10–11 mm. The numbering follows the inner diameter convention used in the Headway family. All VIA models are braided shaft catheters with hydrophilic outer coating and PTFE inner lining.
WEB Deployment via VIA Microcatheters
The WEB procedure requires advancing the VIA microcatheter into the aneurysm sac with the tip positioned at the mid-sac level. The pre-loaded WEB delivery system is then advanced through the VIA, deploying the WEB as the delivery catheter is retracted. The V-Grip handle controls fine WEB positioning adjustments within the sac before the WEB Detachment Controller releases the device.
The VIA’s matched-diameter design ensures that the WEB transitions smoothly from the delivery catheter lumen to the aneurysm sac without the mechanical resistance that would occur if a mismatched microcatheter were used. This smooth transition is important for accurate initial WEB positioning and for reliable resheathing during repositioning manoeuvres.
Clinical Applications
VIA microcatheters are used exclusively for WEB SL and WEB SLS procedures. They are not appropriate for coil delivery and should not be used as general-purpose intracranial microcatheters. Programs performing both coil embolization and WEB treatment should stock VIA and Headway families separately.
The primary WEB indications — wide-neck bifurcation aneurysms at MCA, AComA, ACA, and basilar apex — constitute a substantial proportion of routine aneurysm coiling practice. For high-volume programs treating these locations, VIA 17 (the most commonly used size for 3–7 mm WEBs) should be in standing inventory.
Technique Tips
Confirm WEB sizing from 3D DSA before selecting VIA catheter size. Undersizing the WEB (and thereby the VIA) is the most common cause of inadequate neck coverage. Navigate the VIA to mid-sac level using a 0.014 in wire under road-map guidance, then remove the wire before advancing the WEB delivery system.
After WEB deployment and before detachment, perform a working-projection angiographic run to confirm WEB neck coverage and efferent vessel patency. If coverage is inadequate, resheath with V-Grip and reposition. Do not detach until both coverage and efferent patency are confirmed.
