Cosmos™ Bare-Platinum Coils: Framing and Filling for Cerebral Aneurysm Embolization

The Cosmos™ family is MicroVention Terumo’s premium bare-platinum detachable coil range for endovascular cerebral aneurysm embolization. All Cosmos coils are constructed from pure platinum wire, providing the high radiopacity, softness, and biocompatibility required for intrasaccular deployment in the cerebrovascular circulation. The Cosmos family is divided into two product lines by wire diameter: Cosmos 10 (0.010 in wire) for fine filling of residual void space and small aneurysms, and Cosmos 18 (0.018 in wire) for initial framing of larger aneurysms where a heavier wire provides greater radial force and basket stability.

Bare-platinum coils are the foundational technology of endovascular aneurysm treatment. Unlike hydrogel-coated coils (HydroFrame, HydroSoft) which swell after deployment, bare-platinum coils provide predictable placement and immediate volume filling without volume change over time. Cosmos coils are used as the primary coil for framing and filling, and are frequently combined with hydrogel-coated coils in hybrid protocols that take advantage of platinum’s predictable framing behaviour with hydrogel’s volume-expansion filling benefit.

The Cosmos 10 line serves the filling role for aneurysms of any size — its finer wire allows it to conform to irregular residual void spaces left after framing. The Cosmos 18 line is sized for the framing role in medium-to-large aneurysms (6–24 mm diameter), where the heavier 0.018 in wire provides the radial force needed to form a stable basket against the aneurysm wall.

For aneurysms 3–5 mm in diameter, a typical Cosmos protocol uses Cosmos 10 for both framing and filling: a 3D coil of diameter matching the mean sac diameter for the initial frame, followed by progressively smaller helical coils for filling. The single-wire-class protocol simplifies coil selection and reduces switching between coil lines during the procedure.

For aneurysms greater than 6 mm, a two-class protocol is common: Cosmos 18 3D coil for the initial frame, providing a stable basket with adequate radial force; followed by Cosmos 10 or HydroSoft filling coils to fill the residual volume inside the basket. For giant aneurysms (>25 mm), multiple Cosmos 18 framing passes may be needed before the sac is stable enough for filling coils.

The most widely used coiling protocol at high-volume centres combines Cosmos bare-platinum and MicroVention Terumo hydrogel-coated coils. The standard sequence is: Cosmos 18 for initial framing (stable basket, predictable platinum behaviour), HydroFrame 18 for secondary framing (hydrogel expansion tightens the basket and increases packing density), then HydroSoft 3D or Helical for final filling (ultra-soft hydrogel expansion fills residual voids).

This hybrid approach capitalises on the strengths of each coil type: Cosmos platinum’s predictable initial framing, HydroFrame’s basket consolidation through hydrogel expansion, and HydroSoft’s ultra-soft final filling without the mass effect risk of additional platinum loops.
