Over 25 years ago, Abbott pioneered PFO closure with the Amplatzer™ PFO Occluder. With the introduction of the Amplatzer Talisman PFO Occluder, that proven Amplatzer PFO closure technology has been built upon: the occluder now comes assembled and ready to use, simplifying prep and enhancing ease of use. The 30 mm size has been refined to have a smaller left atrial disc to better avoid interference with surrounding structures. The occluder is offered in four sizes, from a right atrial / left atrial disc combination of 18 mm / 18 mm up to 35 mm / 25 mm, with recommended sheath sizes of 8 F or 9 F (45° curve). Device selection follows the published sizing guidelines: a 25 mm device is suggested for a simple PFO or PFO with a non-prominent ASA; a 30 or 35 mm device for complex PFO with anatomical characteristics such as an atrial septal aneurysm with a long tunnel or thickened septum secundum; and an 18 mm device for PFO with small anatomy (septal primum length < 20 mm). Abbott advises evaluating device position after deployment but before detachment, using echocardiography to ensure the device does not impinge on the free atrial wall or aortic root, and recapturing and redeploying if position is unsatisfactory.
